Penn Home Palliative Care services are designed for patients receiving treatment for a life-limiting condition. We support our patients' medical care by addressing their physical, psychological, emotional, spiritual and social needs.

Our goal is to provide the best in individualized palliative care so patients can remain at home, preserve their independence and adapt to lifestyle changes during this challenging time. Penn Home Palliative Care is designed for patients who are still receiving treatment while hospice is most appropriate for patients who wish to focus on comfort care.

Penn Home Palliative Care offers specialized services such as:

• Help with nausea, fatigue, shortness of breath and anxiety
• Help with understanding your medications
• Physical and occupational therapy to maximize quality of life
• After-hours emergency support via telephone or nurse visit
• Assistance in completing an advance directive or living will
• Support for family and caregivers
