Home health care

High-quality health care in the comfort of your own home helps you stay strong, feel better, and maintain your independence.

Skilled health care at home from Penn Medicine

Recovering from an illness or surgery or living with a chronic condition doesn’t always require a hospital stay. You may be able to get the care you need without leaving home.

We offer convenient, high-quality home care nursing and rehabilitative services to help you recover and remain independent. We are accredited by The Joint Commission and Medicare in recognition of the rigorous quality and safety standards we provide our patients.

Home health care: What to expect

Your healthcare provider makes a referral for you to participate in our home health care services. Once you are approved, we set up a consultation to learn your needs and develop a personalized plan for your treatment. We encourage family and caregivers to attend this consultation to support you.

The number of home health visits you receive depends on your needs. Some people require daily care while others only need help a few days a week. During your care journey, we keep your doctor informed about your progress.
