This program offers members a concierge approach to primary care at Penn Medicine. This innovative approach to health care gives a patient the opportunity to partner with their physician to develop and implement comprehensive health and wellness programs tailored to their short-and long-term goals.
Penn Personalized Care physicians manage a member’s routine, acute and chronic medical problems. Members enjoy a consistently high level of care and service. Benefits include:
- 24/7 access via member phone line and email: Members enrolled in Penn Personalized Care are provided with a members-only phone number, allowing direct access to the practice and doctors 24 hours a day, seven days a week. Members with health questions or concerns after 4:30 p.m. or on weekends may contact the physician directly via their personal cell phone or email.
- Personalized physician wellness visits & health counseling: In addition to preventive care visits, patients benefit from longer wellness-focused counseling sessions. These sessions allow for in-depth discussions on wellness-related concerns and an enhanced assessment that goes beyond a traditional annual examination. Members will have an actionable wellness plan designed to help them meet their personal health goals.
- Same-day/next-day appointments: For member convenience, same-day and next-day appointments are available for individuals requiring prompt medical attention or upon request based on availability. Members who require an appointment for routine exams and do not request same-day/next-day appointments are scheduled within two to four weeks.
- Medical advocacy services: When appropriate, physicians will be directly available by telephone to coordinate your care with other members of your care team, including specialty or other healthcare visits. This ensures that the appropriate exams and screening procedures are taking place as needed and that diet and fitness goals are evaluated.
- Direct access to billing representative: If you have billing issues or questions, members have direct access to our billing representative.
- Enhanced reception area with amenities: To ensure the members comfort and convenience, Penn Personalized Care offers a private reception area with amenities, including refreshments and wireless internet access.
