Improving health care access and navigation

Connecting people to care where they need it most

At Penn Medicine, serving the community is at the heart of what we do. We go beyond hospital walls to meet people where they are and connect them with the resources and support they need. Through strong partnerships and proven strategies, our programs tackle important health and social challenges, ensuring that individuals and families get the care they deserve. By working closely with local organizations, leaders, and residents, we help build healthier, stronger communities.

Our commitment to outreach has led to innovative programs that have inspired similar efforts in cities across the country. These initiatives set new standards for improving access to care, addressing health disparities, and creating lasting solutions to public health challenges. With dozens of outreach programs in place, Penn Medicine continues to lead the way in community engagement.
