Supportive services for older adults at Penn Medicine
Our goal is to optimize your health and prevent unnecessary hospitalizations. Senior HealthLink is available for people age 55 and older in the southeastern Pennsylvania and New Jersey area.
Our services include:
Upper-level nursing students supervised by their nursing instructors, program directors, and nursing staff visit your home to provide services, such as:
- Basic health assessments
- Blood pressure and pulse checks
- Community resource information
- Health education
- Health surveys
- Home safety assessments
- Medication reviews
- Socialization
Our health assessments and educational resources are available by phone to people in the southeastern Pennsylvania and New Jersey area. We also offer a Community Resource Phone Line where you can get referrals and information about local resources for older adults. You can reach us at 610-431-1852.
We teach you how to manage your medications and prevent falls to help keep you safe and healthy.
