Uncontrolled chronic diseases such as high blood pressure (hypertension), heart disease, and diabetes can lead to decreased quality of life and shorter life spans.
We have expanded our quality measurement initiatives to include population-level data from our hospitals and clinics to help identify and address chronic disease rates.
High blood pressure rates in Philadelphia
This map and table show the percentage of Penn Medicine patients age 18 to 85 in certain Philadelphia zip codes who had high blood pressure (hypertension). Recently, the definition of hypertension shifted to include patients who have multiple readings greater than or equal to (≥) 130/80. Previously, hypertension was defined as ≥ 140/90. Here, we are sharing our rates of hypertension using ≥ 130/80 and are including rates for our primary care patients. We will update this information to reflect the latest data as our research continues.
