Palliative care services we provide
Our skilled care team helps people of all ages by providing the support, care, and equipment you need to stay in the comfort of your own home. Our palliative care services include:
A registered nurse visits you regularly to help relieve symptoms such as pain, nausea, shortness of breath, trouble sleeping, and anxiety. We offer 24/7 support to make sure you stay comfortable.
We can help you understand your medications and remember when to take them for the most effective symptom relief.
Physical and occupational therapists help you adapt to physical changes, increase your mobility and independence, and keep you comfortable.
Our social workers are available to talk about how you’re feeling and help you develop healthy coping strategies.
A home health aide helps you with daily activities, such as eating, bathing, and dressing.
We connect you to free legal services through the Hospice Legal Project. The program refers you to a SeniorLAW Center attorney, who can help with:
- Personal planning: Wills, inheritance questions, life insurance policies, powers of attorney, living wills, probate questions, funeral planning
- Family issues: Custody, child/spousal support, divorce, domestic abuse, guardianship, stand-by guardianship
- Benefits and pensions: Social Security benefits, Medicare/medical assistance (including medical assistance estate recovery), pensions, Veteran's benefits
- Employment: Unemployment, disability, employment rights, discrimination
- Housing and long-term care: Deed questions, general housing issues (including landlord/tenant problems and delinquent real estate taxes), long-term care questions, discharge rights
