Home health care: What to expect
Your healthcare provider makes a referral for you to participate in our home health care services. Once you are approved, we set up a consultation to learn your needs and develop a personalized plan for your treatment. We encourage family and caregivers to attend this consultation to support you.
The number of home health visits you receive depends on your needs. Some people require daily care while others only need help a few days a week. During your care journey, we keep your doctor informed about your progress.

Penn Medicine’s Hospital at Home program provides hospital-level care in the comfort of your home, supported by a dedicated team of doctors, nurses, and healthcare professionals. Patients receive 24/7 access to their care team, in-home nurse visits, virtual doctor check-ins, continuous monitoring, and essential services like labs, imaging, and IV therapies.

If you’re recovering from surgery or dealing with a chronic health condition, our registered nurses (RNs) and licensed practical nurses (LPNs) can help you manage medications, pain, wound care, catheter maintenance, and more.
Our physical therapists help you regain mobility, improve muscle strength, relieve pain, and properly use medical equipment after a surgery or injury.
Occupational therapists help you gain the strength and skills you need to perform everyday activities, such as bathing, dressing, and preparing meals.
If you are recovering from a condition such as a stroke, a speech therapist helps improve your ability to communicate and swallow.
Dealing with an illness or accident can impact your mental health. Our medical social workers help you cope with emotional stress. They can also connect you with financial assistance and community resources, such as Meals on Wheels.
If your medical condition requires regular infusions, your nurse can provide IV therapy at home. This involves administering fluids or medication into your bloodstream through a vein.
We provide peace of mind by monitoring your health condition 24/7. We set up a monitor in your home to track vital signs such as blood pressure, heart rate, and weight.
We make sure you and your loved ones have the information and tools you need to feel better, heal, and live independently. We teach you how to perform rehabilitative procedures, manage medications, and make lifestyle changes that support your health.

Penn Medicine accepts Medicaid, Medicare, Medicare Advantage, worker’s compensation, and private insurance plans. Our staff can help you understand your plan’s benefits and any charges you may receive. Financial assistance is available, if needed.

Service availability
Penn Medicine Home Health provides the following services: skilled nursing, physical therapy, occupational therapy, speech-language pathology, home health aide, and social worker. To qualify for home health services, a patient must be under the care of a physician or allowed practitioner who is responsible for the home health plan of care.
Service limitations
Penn Medicine Home Health (PMHH) does not provide services for patients under 18 years of age, maternal/obstetrical/post-natal services, psychiatric/behavioral health services, and does not provide care for patients who we are aware of are actively using or are under the influence of drugs or alcohol. Penn Medicine Home Health does not provide care where equipment required exceeds the competence of our staff including but not limited to ventilators. We cannot provide care if our staff is required to manipulate or manage specialized equipment, and staff is not trained in the use of the specialized equipment. PMHH does not provide care for patients who have been in violation of the UPHS Code of Conduct, including those that have a history of violence, abusive or threatening behavior towards staff. PMHH does not provide service for patients who require more than once per day nursing visit and/or when there is no caregiver available to provide care. Our services are provided in the following counties: Philadelphia County, Chester County, Montgomery County (limited zip codes), Berks County (limited zip codes), Bucks County, Delaware County, and Lancaster County (limited zip codes).
