When you are ready to enroll, a dedicated coordinator can help you navigate the process and gather necessary health information so you can start experiencing the benefits of Passport membership as soon as possible.
Annual memberships are available to individuals, couples, families, and corporations. Family membership includes the member, their spouse or significant other, and dependent children between 18 and 26 years old who live in the same household.
The Passport program is a health care advocacy and care coordination and facilitation program. Among many benefits, a Personal Care Coordinator can help schedule your appointments, arrange for someone to meet and escort you, and help coordinate additional services at all Penn Medicine locations, as needed.
We have a tiered pricing model that is dependent on the number of members covered (individual/couple/family/corporation). To learn more, call us at 215-615-2055 or email pennpassport@pennmedicine.upenn.edu.
