According to the Affordable Care Act, all U.S. citizens must have health insurance coverage or pay a penalty. Based on household income and dependents, you may be eligible for insurance at low or no cost. Health insurance plans in the Marketplace offer comprehensive coverage, from doctors to medications to hospital visits. Some plans provide only dental benefits.
Marketplace plans are called qualified health plans (QHPs). You can compare all your insurance options based on price, benefits, quality, and other features that may be important to you, in plain language that makes sense.
Certified Application Counselors (CAC)
If you are uninsured, our Certified Application Counselors (CAC) can assist you in understanding your medical insurance coverage options and help facilitate enrollment during the open enrollment period.
There is no fee to meet with a CAC.

To complete your application, you will need:
- Social Security numbers or document number for legal immigrants
- Employer and income information for every member of your household who will need coverage (pay stubs or W-2 forms)
- Policy numbers of any current health insurance plans covering members of your household
- A completed employer coverage tool for every job-based plan you or someone in your household is eligible for (You will need to fill out this form even for coverage you are eligible for but do not enroll in. You can find it at healthcare.gov)

Note that in some cases, plans may not offer coverage for care provided at Penn Medicine locations. Other plans may offer tiered benefits, with higher out of pocket costs associated with some providers. All patients should consult their insurance plan resources to determine different levels of co-pays, deductibles, co-insurance and maximum out-of-pocket limits when seeking care at Penn Medicine locations.
