Street medicine in Lancaster County
The Lancaster General Health Street Medicine, led by Director Jared Nissley, MD and Bianca Cruz, DNP, CRNP aims to improve the health of unhoused individuals in County. We deliver primary, acute, and social care through our mobile practice. Our team visits shelters, day centers, soup kitchens, and encampments multiple days each week to help people access the care they need.
At the core of our program, we value:

- Honoring each individual’s autonomy over their care
- Listening to the input of those with lived experience
- Meeting people where they are
- Practicing with a flexible, reality-based approach
Primary and acute care services
We offer medical care for a variety of conditions, including:- Chronic diseases such as diabetes or high blood pressure
- Infections
- Mental health concerns, such as depression, anxiety, or substance use
- Musculoskeletal complaints
- Skin problems
Our team helps coordinate referrals to more advanced specialty care as needed.
Social services
We work closely with outreach coordinators, substance use peer workers, social workers, and other community experts to help people get necessary social services. Our care also includes:
- Conversations about housing and connections to resources
- Discussions about benefits management
- Distribution of necessary supplies, such as medications, blankets, and other needed survival items
