Shreya Kangovi, MD, MSHP, Assistant Professor of Medicine and Executive Director, Penn Center for Community Health Workers, created IMPaCT, a comprehensive, evidence-based Community Health Worker model. Dr. Kangovi‘s research examines patient perspectives on socioeconomic determinants of health and healthcare utilization, and the efficacy of community health workers (CHWs) in modifying these determinants. She led a multi-stakeholder team that designed a CHW intervention (IMPaCT). This intervention has been tested in two randomized controlled trials and shown to improve chronic disease control, primary care access, mental health and quality of care while reducing hospital admissions. Dr. Kangovi founded the Penn Center for CHWs to support further research, development and implementation of evidence-based CHW models of care. The center delivers the IMPaCT intervention to 2,000 patients per year at Penn Medicine and supports health care organizations across the country in developing effective, scalable CHW programs.
