Patient Navigators are committed to improving the experience of our patients and families. Navigators are experts in navigating complex health care situations. They can help in a variety of ways including:
- Helping to connect patients to the appropriate providers
- Facilitating access to information
- Offering clarity to the plan of care
- Referring patients and families to support services
Cancer patients and their families deserve quality care and services as they fight their disease. Patient navigation services can provide individualized support, care coordination, empowerment and advocacy to ensure your needs are met. Patients often need one "contact" person or an identifiable team they can call when they have a problem or a need. Patient navigators fill this role.
