To schedule a consultation
A patient should be considered for transplant evaluation at the time of referral to dialysis, or when the patient's GFR is less than or equal to 20 ml/min. For additional information please refer to the program selection criteria.
To begin the evaluation process, patients can call 215-662-6200, or fax the referral form to the kidney transplant program. A patient care representative will call patients to schedule appointments.
To facilitate the appointment process, it is helpful for patients to have the following information available:
- Full name, address and phone number of their referring nephrologist (kidney doctor)
- Name, address and phone number of dialysis unit
- Full name, address and phone number of primary care physician
- Full name, address and phone number of any other specialists
- Insurance information
Please note, to serve patients in central and northern New Jersey, kidney transplant evaluations are offered at Penn Medicine Bucks County, in Yardley, PA. To serve patients traveling from north and west of Philadelphia, kidney transplant evaluations are offered at Penn Medicine Radnor in Radnor, PA.

Once the evaluation appointment has been scheduled for dialysis patients, the following information is requested from the dialysis unit:
- End-Stage Renal Form 2728
- Most recent history and physical
- Pathology report
- All documentation regarding nutrition, social work, and finance
- Hematology and oncology records (past two years)
- All labs (ABO/PSA) (past three months)
- Cardiac studies (past two years)
- GI/urology consults (past two years)
- Nephrologist notes
- Office notes
- Kidney biopsy results
- Kidney ultrasound (most recent)
- PPD results/immuno (most recent)
- Rounding report (past three months)
- HP (initial/most recent)
- EGDcColonoscopy (past 10 years)
