Before leaving the hospital, your physician may request your admission to the Skilled Care Center at Pennsylvania Hospital. The skilled care services at Pennsylvania Hospital are designed to meet the special needs of people who no longer require acute hospital care, but who still need medical attention until they are ready to go home.

Staff and Service
Our staff consists of professionals from nursing, rehabilitation and other fields who take a team approach to patient care. They work closely with you and your family to develop the treatment and discharge plans best suited to your individual needs, and can provide any services a patient in a hospital setting might require, including laboratory, radiology and respiratory therapy.

An Atmosphere for Recovery
Patients will benefit from a relaxed and informal environment, with a warm, attractive décor. The focus of your stay is on gaining independence and getting well. Residents are encouraged to wear comfortable street clothes and sturdy shoes during therapy.

The following amenities are available to all residents:

  • A shower in every room
  • One specially equipped shower on the unit for people recovering from a serious illness or major surgery
  • Laundry facilities
  • Lounge for activities and visiting

Residents of the center may eat in the privacy of their rooms or eat with a group of residents in the dining areas. Your family and friends are encouraged to visit during the center’s extended visiting hours.

Important components for recovery are activities that promote physical and mental well-being. A variety of daily social activities are offered, including:

  • Movies
  • Special dinners
  • Horticulture activities
  • Music
  • Bingo
  • Board games and puzzles

Most activities take place with groups in the center’s lounge, but individualized activities can be arranged at your bedside if you are unable to attend.

Accreditation
The Skilled Care Center at Pennsylvania Hospital is licensed as a long-term care facility by the Pennsylvania Department of Health and is accredited by the Joint Commission on Accreditation of Health Organizations for sub-acute care.

Admission
A skilled care case manager will meet with you in the hospital and determine if admission to one of our skilled care facilities is appropriate for your needs. The case manager will discuss with you and your family the goals of your stay at the center and about preparing to go home.

Feel free to ask the case manager about:

  • Insurance coverage or other financial arrangements
  • Referrals to appropriate community services

The case manager can provide any other assistance that will make your stay at the center and your transition to home easier.

You will be admitted after your discharge from the hospital. Your length of stay depends upon the speed of your recovery and the recommendations of your physician and the center’s health care team.
