When physical limitations make a visit to one of our practices impossible, our house calls program offers patients the exceptional care of Penn Medicine geriatrics in their own home. Nurse practitioners and physicians provide complete primary care for the homebound patient on a temporary or permanent basis. Care coordination with additional agencies, medical equipment and social work is led by experienced geriatric medicine practitioners to maintain successful living at home.
