Affiliation Agreement

Please confirm with your school's Program Director or Office of Student Affairs that a current Affiliation Agreement is in place and will remain current beyond the dates of the proposed student rotation.
A contract must be specific to the entity (Hospital of the University of Pennsylvania, Penn Presbyterian Medical Center, Pennsylvania Hospital, or Good Shepherd Penn Partners, and Penn Care at Home) and specific to either Undergraduate and/or Graduate programs. These contracts define the requirements that the Accredited School must meet specific to student health screening and immunization status, certification, insurance coverage, liability, drug screening, and criminal background check.
