Notice for Patients Seen at Penn Presbyterian Medical Associates (3801 Filbert Street in the Medical Arts building) between 2003 and 2010
On September 15, 2012, Penn Presbyterian Internal Medicine Associates became aware that an unencrypted computer hard drive was stolen from the office of Dr. Joel Griska, a physician of the Practice. The drive contained documents related to care that was provided to certain patients between 2003 and 2010. The documents contained patients' full name, date of birth, contact information (including phone number and address), visit summary/physician notes, diagnosis information, and medication lists.
The theft was reported to the appropriate authorities, but the computer drive has not been located to date. At this point, we have no reason to believe that the information contained on the drive has been accessed and/or used inappropriately. However, we felt it was prudent to inform individuals of the theft since certain information contained on the stolen drive, if inappropriately accessed, could potentially expose such individuals to reputational, financial or other harm. Notification has been mailed to all affected individuals, but because of out of date and/or insufficient contact information for some of our patients, we are providing this alternative notification.
